For horror:
No Escape: Male Rape in US Prisons (http://www.hrw.org/reports/2001/prison/report.html)
For worse:
Stop Prisoner Rape (http://www.spr.org/)
As for the assertion that nobody has been exonerated after execution, I'd suggest that the assertion is quite true - and hellaciously misleading.
To quote the DPIC (http://www.deathpenaltyinfo.org/innocothers.html#executed) "There is no way to tell how many of the over 750 people executed since 1976 may also have been innocent. Courts do not generally entertain claims of innocence when the defendant is dead. Defense attorneys move on to other cases where clients' lives can still be saved."
So how are the incorrectly executed to be exonerated?
Even so, the site lists a large number of cases where subsequent evidence indicates that the wrong person was executed.
